A Rational Periodontal Surgical Approach for Crown Lengthening in the Esthetic Zone: Case Report


Summary

The term "gummy smile" is used to describe a relatively common condition characterized by excessive gingival exposure of the upper arch during smiling. The presence of thick and fibrous tissue may contribute to the delayed migration of supracrestal tissues during the passive phase of dental eruption, resulting in gingival overlapping the enamel boundaries and, consequently, crowns with a square and short appearance. The repositioning of gingival and bone margins through periodontal plastic surgery promotes the full exposure of the anatomical crown and the reduction of gingival exposure, leading to an improved esthetic outcome. The conventional surgical technique for clinical crown lengthening prescribes the leveling of gingival margins through submarginal incisions followed by the elevation of a full-thickness flap for osteotomy. This case report presents an alternative approach, where flap elevation and osteotomy are performed prior to submarginal incisions, thus achieving greater precision regarding gingival margin healing.
